- Call ahead: Call (616) 432-3103 and our office managers will email or text you your adult or pediatric intake form to complete before your visit.
- Arrive 15 minutes early: Prefer paper? Just arrive a little early to your first visit and we will have everything ready for you at the front desk.
What to bring
- A valid photo ID and your insurance card, if applicable
- A list of current medications
- Any prior imaging or records related to your condition, if you have them
